The first chapel on this site was built by José de Miranda between 1700 and 1720, and it was established as a parish in March 1740 under the patronage of the Immaculate Conception of Mary. It went from rural chapel to parish and, centuries later, to cathedral seat.
Pope Paul VI created the Diocese of Carapeguá on June 5, 1978, elevating the church to cathedral status. A full renovation, begun in 1986, reopened in December 1993. It stands in the center of Carapeguá, in the Paraguarí department.
